What to Expect
Thank you for choosing DePaul Community Health Centers for all your health care needs. Our goal is to keep you healthy, not just treat you when you are sick. During every visit, a team of experienced health care professionals considers your overall health and works with you to develop a plan to help you stay as healthy as possible. You can expect:

Same-day appointments when you are sick

Health care you can afford
Your doctor is the leader of your health care team, and he/she will see you every time you visit.
If your doctor recommends a test or service not available at one of our four health centers, your Care Coordinator will arrange for you to receive tests or services at another facility.
If you need help managing a chronic condition, your doctor may assign a Care Manager who will:

Give you information and tips to help avoid complications

Teach you new skills to help manage your condition
Your Care Management Team includes a Nurse, a Pharmacist and/or a Social Worker.
Payment Information
All DePaul Community Health Centers proudly accept Commercial Insurance, Medicaid and Medicare.
Patients that qualify for our sliding fee scale will be required to pay between $35 and $95 for medical and behavioral health visits (depending on your financial status). Dental patients will be required to pay $55 for an initial exam and screening and other services will be provided at the cost identified. Patients will qualify for a discount if they meet the income requirements and provide the appropriate documentation (listed above). Payment is expected at the time of the visit. Additionally, all reference labs and invasive procedures will have a separate fee that must be paid prior to the lab or procedure. Please note that reference labs are labs that we send to an external company (such as Lab Corp or Quest Diagnostics).
Self-Pay/Uninsured dental patients who qualify (those whose household income is between 100% and 200% of Federal Poverty Guidelines) pay a nominal $55 fee for dental visits, which include an exam, x-ray and treatment plan).
We also would like to remind you that we are a Medicaid and Affordable Care Act enrollment site.
Lastly, please note that we accept all insurance plans, including: Blue Cross Blue Shield, United Health Care, Humana, Peoples Health, Cigna, Aetna, Amerihealth Caritas, Amerigroup, Louisiana Healthcare Connections.
What to Do Before Your Appointment

Confirm the date, time and location of your upcoming appointment

Make a list of all of your medications to share with your doctor

Locate your most recent insurance card and prepare to bring it with you to your visit

Plan to arrive 30 minutes early for your first appointment

In the event that you can no longer keep your appointment, contact us at least 24 hours in advance to reschedule

DePaul Community Health Centers is committed to improving the health status of our community. We continue to advance our long tradition of health ministry through the development of a values-driven organization dedicated to primary care and preventive health services, addressing the needs of the total individual – body, mind and spirit.
We provide discounts to uninsured patients based on family size and household income. In order to comply with federal healthcare regulations and guidelines, we are required to collect proof income from all patients that wish to qualify for our sliding fee scale.
Please note that the following documents are the only documents that we will be accepting as proof of income.

Payroll Check Stubs (1 to 2 most recent, must be within last 30 days)
